What to Expect

Expect to notice some swelling. Fortunately, the face puffiness will gradually subside.

While swelling will peak around the first three days, it’ll be 80% gone by the end of two weeks, and 90% will have resolved by two months.

Cutaneous facelift:

Also known as a “skin-only” rhytidectomy, this type of facelift, as its name suggests, focuses solely on tightening the skin around the lower face and neck.

In contrast, a traditional facelift procedure involves the skin’s underlying tissues, such as the muscles and other supporting facial and neck structures.

Unfortunately, if only addressing the skin and not tightening the underlying structures, expect more skin tension. This creates wider scars, shorter lasting results, and does not address neck sagging or jowling very effectively, which may create a lateral sweep appearance.

SMAS rhytidectomy:

This type of facelift, which provides a more comprehensive lift, targets the muscular layer beneath the skin, which is known as the superficial musculoaponeurotic system (SMAS).

It addresses sagging in the lower two-thirds of the face, including the cheeks, jowls, and jawline, by tightening the SMAS layer and trimming the excess skin and fat.

Unlike skin-only facelifts, SMAS rhytidectomy offers more natural and longer-lasting results. Thus, it’s an excellent option for achieving a more youthful appearance without the outcome looking “overdone.”

Because the deep attachments of the SMAS are not released, the amount of SMAS movement and therefore final results may be more conservative.

Deep plane facelift:

This type of rhytidectomy lifts the SMAS, skin, and fat as a single unit. This approach allows for a more thorough repositioning of the facial structure by releasing the deeper attachments to the SMAS. 

This technique allows maximal vertical movement of the SMAS and powerful improvement in the jowls and neck. This is the favored technique of Dr. Rihani as he can create a more natural improvement in the neck and jawline while also improving the nasolabial folds and marionette lines. 

Mini facelift:

Although not considered a true facelift technique, this term is commonly used to imply less downtime or shorter incisions. Dr. Rihani considers this more to be a marketing term that can imply a more “minimally invasive” procedure and can mean different things to different surgeons.

Short-scar facelift:

As its name implies, a “short-scar” rhytidectomy only involves a short incision. This is not an actual technique, but rather describes that a scar will be shorter than typical incisions. This type of incision is great for patients that have minimal amounts of skin needing to be removed.

Lift-and-fill facelift:

This procedure involves a rhytidectomy combined with fat transfer/grafting. Its primary benefit is that it can address tissue laxity and volume loss.

Some of the most common nonsurgical facelift procedures are the following:

  • Dermal fillers: This less invasive solution involves injecting substances, like hyaluronic acid, calcium hydroxylapatite, and poly-lactic acid, to add more volume to the cheeks, chin, and jawline.

  • BOTOX®: Also known as botulinum toxin, BOTOX® works by relaxing the muscles, reducing wrinkles caused by muscle contraction, including “frown lines” and “crow’s feet.”

BOTOX® can give a relaxed, youthful appearance through a simple injection.

  • Thread lift: Like a rhytidectomy, thread lifting aims to “lift” sagging tissues to restore them to their original, younger positions. The former accomplishes this goal via incisions and skin removal.

However, the latter doesn’t. Instead, thread lifting uses, as its name implies, barbed sutures or “threads” to pass through the skin, attach to soft tissues, and suspend the skin to a more superior location. Unfortunately, the results of these threads can be quite short lasting and many patients are disappointed a 6 months to a year after the procedure.

So, should you opt for a nonsurgical facelift? Unfortunately, there are various reasons you shouldn’t, despite its ability to give you the appearance you want without the extended downtime.

For one, nonsurgical options like thread lifting don’t come close to providing the results that a surgical rhytidectomy can. After all, they don’t address the SMAS layer, a significant component of the facial structure.

Another reason is that minimally invasive facelift procedures are stop-gap measures.

For instance, the effects of BOTOX® typically only last three months, making keeping a youthful appearance costly due to the required follow-up treatments.

  • Stopping certain medications: Some medications, such as anti-inflammatory drugs, can increase bleeding and impact recovery. For this reason, we’ll ask you to stop taking them.
  • Curbing bad habits: Habits like smoking cigarettes or vapes can impact a facelift outcome. We advise our patients to stop smoking or any nicotine products at least three months before their procedure.
  • Relaxing: A surgical operation like a facelift can be anxiety-inducing. For this reason, we advise our patients to take some time to pamper themselves, particularly the night before the surgery.
  • Staying hydrated: Proper hydration makes anesthesia safer and easier for you and our medical team. So, you should drink at least 60 ounces daily in the days leading up to your procedure.
